POSITION TYPE: Intake Coordinator

LOCATION: Bingham Farms, MI

SHIFT: Days 9-5pm, No Weekend

JOB TYPE: Full-Time Permanent

Home health nursing plays an important role in the healthcare field. This form of nursing offers a high level of independence and dignity to patients and their family members. Over 7 million people in the U.S. require the services of a home health nurse due to a permanent disability, acute illness, terminal illness, or long-term health issue. As an Intake Coordinator you will:

• Receive referrals and advise patients, family members, staff, and physicians about programs available for patient care needs.
• Document referral orders with signature and date; ensures orders received by LVN/LPN are reviewed and cosigned by RN.
• Comply with the applicable legal requirements, standards, policies and procedures including, but not limited to the Compliance Program: Code of Conduct, HIPAA and Documentation Standards.

Requirements:

• Licensed Vocational Nurse with current state license or non clinical is oaky
• Minimum of one (1) year experience in home care preferred.
• Prior use of Electronic Medical Records (EMR) preferred.
• Knowledge of state, federal, and local regulatory requirements.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. 
• Ability to travel to all business locations
