Job Title: Registered Nurse Radiology/ Pain Management
Department: Nursing Administration
Schedule: Full-Time, Day Shift (Monday-Friday, 9:00 AM – 7:00 PM).
Position Summary:
Seeking a compassionate and experienced Registered Nurse to join our dynamic Discharge team. In this role, you will play a vital role in ensuring a smooth and safe transition for patients as they prepare for discharge. You will collaborate closely with patients, families, physicians, and other members of the healthcare team to provide high-quality, patient-centered care.
Key Responsibilities:
- Assess patient needs and develop individualized discharge plans.
- Provide education to patients and families regarding medications, wound care, diet, and other discharge instructions.
- Coordinate discharge transportation and home health services as needed.
- Address patient concerns and provide emotional support during the discharge process.
- Monitor patient vital signs and assess for any potential complications.
- Maintain accurate and timely documentation in the electronic medical record.
- Collaborate effectively with the interdisciplinary healthcare team.
- Adhere to all hospital policies, procedures, and safety protocols.
- Participate in quality improvement initiatives and professional development activities.
Qualifications:
- Graduate from an accredited school of nursing with a current New York State RN license.
- Bachelor of Science in Nursing (BSN) required.
- One year of acute care nursing experience preferred.
- BLS certification required.
- ACLS certification required (within 12 months of hire for new hires).
- Excellent communication, interpersonal, and patient education skills.
- Strong organizational and time-management skills.
- Ability to work independently and as part of a team.
