Job Title: Master’s-Level Social Worker

Job Summary:

The Master’s-Level Social Worker provides skilled interventions to support patients and families in managing complex psychosocial and socioeconomic challenges. Responsibilities include assisting with crisis resolution, problem-solving, decision-making, advocacy, and navigating life-changing events such as adoption, guardianship, abuse, bereavement, or illness. The social worker helps mitigate risk factors, promotes effective coping strategies, and ensures that all services are delivered in compliance with relevant professional standards and institutional policies regarding confidentiality, patient rights, infection control, and safety.

Key Responsibilities:

  • Respond promptly to referrals and assess patients, families, and significant others to determine psychosocial needs.
  • Develop and integrate social work plans into overall patient care in collaboration with the interdisciplinary team.
  • Provide comprehensive case management, discharge planning, continuing care coordination, crisis intervention, and patient/family education using individual and family modalities.
  • Address issues related to abuse and neglect, advance directives, psychiatric concerns, and end-of-life planning.
  • Utilize hospital, community, and governmental resources to support patients and families; advocate for access to needed services and benefits.
  • Refer patients, families, and staff to appropriate internal or external services to ensure quality and continuity of care.
  • Maintain knowledge of resources specific to various patient populations and facilitate access to insurance coverage and funding sources for necessary care.
  • Document care plans, interventions, and outcomes accurately and promptly in the medical record.
  • Complete required reports and documentation in accordance with departmental or regulatory standards.
  • Collaborate with the interdisciplinary team to develop and implement complex discharge and transitional care plans, including placement in long-term care or community facilities.
  • Engage in ongoing professional development through supervision, training, and participation in educational programs to enhance clinical skills, cultural competence, and self-awareness.
  • Participate in community education and support activities as assigned or approved.

Decision-Making:

  • Exercises independent judgment and decision-making within established professional standards, with review primarily in final stages.

Working Relationships:

  • Collaborates and communicates effectively with patients, families, and other healthcare professionals.
  • Works directly with both internal and external stakeholders, including healthcare staff, community agencies, and facility leadership.

Required Skills & Experience:

  • Prior experience working with the assigned patient population.
  • Computer literacy and ability to document effectively in electronic systems.

Preferred Skills & Experience:

  • Experience in hospital or community health settings.
  • Fluency in an additional language beyond English.

Education Requirements:

  • Master of Social Work (MSW) or a closely related degree from an accredited program.

Licensure & Certification:

  • Preferred: Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC).
