Nephrology Nurse Practitioner – Houston, TX

We are currently seeking a compassionate APRN to join an established nephrology practice in the Houston area.

This is a full-time opportunity .

The Opportunity

  • Full-Time | Monday–Friday
  • 40 hours per week
  • South East Houston Outpatient Dialysis Clinical Setting
  • Inpatient and outpatient nephrology responsibilities
  • Collaborative environment with supervising Nephrologists

Responsibilities

  • Conduct dialysis rounds at multiple dialysis units.
  • Perform comprehensive access assessments and management.
  • Prescribe and adjust dialysis-related medications as appropriate.
  • Assess and establish parameters, including target weight and dialysis prescriptions.
  • Perform comprehensive history and physical examinations on new and established dialysis and AKI patients.
  • Initiate appropriate dialysis orders, treatments, and diagnostic testing as needed.
  • Order therapeutic and diagnostic procedures within scope of practice and training, as directed by the supervising physician.
  • Provide ongoing follow-up and health maintenance care for dialysis patients according to established protocols.
  • Provide patient and family education regarding health-related conditions and treatment plans.
  • Document clinical care in accordance with established guidelines.
  • Communicate effectively with patients and family members.
  • Consult with the supervising physician as needed.
  • Provide outpatient office/clinic visits focused on the diagnosis, treatment, and prevention of renal disease and management of CKD patients.

Requirements

  • Current Nurse Practitioner license and credentials.
  • Nephrology, dialysis, or renal care experience preferred.
  • Strong clinical assessment and patient management skills.
  • Ability to work independently while collaborating effectively with Nephrologists and the healthcare team.
  • Excellent communication and interpersonal skills.
