Position Overview
We are seeking a compassionate and clinically skilled Registered Nurse (RN) to provide care for patients receiving General Inpatient Hospice (GIP) services. This role focuses on patients with complex or uncontrolled symptoms who require a higher level of nursing care for symptom stabilization and comfort.
The GIP RN will assess patient needs, manage pain and other end-of-life symptoms, administer medications and treatments, and work closely with the interdisciplinary hospice team. The goal is to provide exceptional comfort-focused care while supporting patients and their loved ones through the end-of-life journey.
Key Responsibilities
- Conduct thorough nursing assessments for patients receiving GIP hospice services.
- Monitor and manage complex end-of-life symptoms such as pain, shortness of breath, anxiety, agitation, nausea, and other sources of discomfort.
- Develop, implement, and reassess individualized plans of care based on patient needs and goals.
- Administer medications, treatments, and therapies as prescribed, with an emphasis on effective pain and symptom control.
- Evaluate patient responses to medications and interventions and promptly report changes in condition.
- Provide skilled nursing care based on each patient's clinical status and established goals of care.
- Educate patients and families about disease progression, medications, symptom management, and what to expect during the dying process.
- Offer compassionate emotional support and a calm, reassuring presence to patients and their families.
- Partner with physicians, nurse practitioners, hospice aides, social workers, chaplains, and other members of the interdisciplinary team.
- Communicate changes in condition, symptom concerns, and evolving care needs to the appropriate members of the care team.
- Complete accurate and timely clinical documentation while following applicable policies and regulatory requirements.
- Participate in interdisciplinary team meetings and contribute to ongoing care-plan development and evaluation.
- Support transitions of care when a patient's symptoms stabilize or their level of care changes, including discharge from GIP services when appropriate.
- Promote patient safety, infection prevention, privacy, and confidentiality.
- Provide clinical direction, education, and support to hospice aides, nursing staff, and other caregivers as appropriate.
- Follow applicable hospice, nursing, facility, and regulatory standards.
- Perform other related duties as needed.
Qualifications
- Current, unrestricted Registered Nurse (RN) license in the state of practice.
- Graduate of an accredited nursing program.
- Current BLS certification.
- Hospice, palliative care, acute care, or similar clinical nursing experience preferred.
- Experience with pain management and complex symptom management preferred.
- Strong clinical assessment, critical-thinking, and communication skills.
- Ability to collaborate effectively with patients, families, providers, and interdisciplinary team members.
- Demonstrated commitment to compassionate, respectful, and patient-centered end-of-life care.
- Ability to remain composed, professional, and supportive in emotionally challenging and rapidly changing situations.
Preferred Experience & Credentials
- Prior hospice or palliative care experience.
- CHPN certification or willingness to pursue certification.
- Experience caring for patients with complex or rapidly changing symptom-management needs.
- Experience with IV medications, continuous medication infusions, respiratory symptom management, and other advanced comfort-care interventions.
- Understanding of hospice philosophy, Medicare hospice requirements, and best practices related to end-of-life care.
