As a Home Health Physical Therapist Case Manager, you’ll work alongside other clinicians to provide skilled services that evaluate and address the needs of individuals in their homes. Your goal is to help patients live safely and productively within their home environment. Your focus will be to restore function, prevent or minimize disability following illness or injury, and help clients reach their maximum performance and independence. You’ll evaluate and treat patients using medically prescribed physical therapy programs and may also supervise Physical Therapy Assistants (PTAs) in accordance with agency and regulatory standards.
Job Description:
- Perform evaluations and develop individualized care plans, collaborating with PTAs to ensure continuity of care.
- Assist patients with mobility, movement, and regaining their independence.
- Create detailed treatment plans for each patient and adjust as needed.
- Communicate and coordinate with physicians to ensure quality care.
- Evaluate and report patients’ progress to the referring physician.

Requirements:
- Current, unrestricted CA Physical Therapist License
- Current BLS/CPR Certification
- Valid CA Driver’s License
- Knowledge of local community resources is required
- Understanding of individual development and human behavior, including the effects of illness/injury and the influence of culture on healthcare
- Knowledge of state and federal homecare regulatory guidelines preferred
- Home health care experience is highly preferred
- Experience with high-acuity patients is a plus
- Interdisciplinary team experience is highly desired
- Case management knowledge is highly desired
- Strong critical thinking, interpersonal, leadership, and organizational skills
