Job Description:

We are seeking a compassionate, experienced, and dedicated Nurse Practitioner to join our Outpatient Palliative Care team. This role is pivotal in delivering individualized care to patients facing serious or terminal illnesses. The ideal candidate will have a strong background in acute care, hospice, or palliative care, and be capable of working independently across a variety of care settings including home visits, extended care facilities, telehealth, and clinic-based care.

Key Responsibilities:

• Provide comprehensive palliative care services across a variety of outpatient settings.
• Manage a daily caseload of approximately 6–8 patients, with travel across a 9-county area in Northwest Ohio.
• Engage in sensitive goals-of-care and advance care planning discussions with patients and families.
• Evaluate, plan, and implement individualized care in alignment with palliative principles.
• Collaborate with a highly supportive interdisciplinary team, including physicians, NPs, and administrative personnel.
• Participate in transition-of-care planning, identifying when patients require ongoing palliative care or transition to hospice services.
• Work in close coordination with a hospitalist team responsible for inpatient admissions.
• Document all patient encounters in accordance with organizational standards and maintain open communication with care teams.

Minimum Requirements:

• Board Certified Nurse Practitioner – Required
• Active Ohio State License – or willing to obtain – Required
• Experience as an advanced provider in an acute care, palliative care, or hospice setting – Required
• (New graduates are not eligible for this role.) Experience as an NP is required.
• Clean malpractice history and active, unrestricted licensure – Required
• Reliable personal transportation – Required (travel reimbursed)
