Position Summary:

The Clinical Pharmacist – Bone Marrow Transplant (BMT) is a key member of the interdisciplinary transplant team at an acute care hospital, providing advanced clinical pharmacy services to adult and/or pediatric patients undergoing hematopoietic stem cell transplantation. This pharmacist ensures the safe, evidence-based, and cost-effective use of high-risk and supportive care medications throughout the transplant continuum. The role also supports research initiatives, protocol development, and education within the BMT program.


Key Responsibilities:

  • Collaborate with hematology/oncology and transplant providers to develop individualized treatment plans for pre-transplant conditioning, immunosuppression, antimicrobial prophylaxis, and post-transplant care.
  • Monitor patients for therapeutic response, toxicity, drug interactions, and organ function during all phases of transplant care (pre-, peri-, and post-transplant).
  • Provide pharmacokinetic dosing and adjustments (e.g., busulfan, vancomycin, aminoglycosides) using evidence-based protocols.
  • Participate in inpatient rounds and ambulatory transplant clinic visits as part of a multidisciplinary team.
  • Conduct medication reconciliation and counseling for transplant recipients and caregivers.
  • Contribute to transplant-related policy and guideline development, clinical pathways, and order set maintenance.
  • Serve as a resource for investigational drug protocols, including handling, documentation, and compliance with research regulations.
  • Precept pharmacy residents and students with a focus on oncology or transplant.
  • Participate in quality improvement initiatives, including medication safety and outcomes monitoring within the BMT program.
  • Stay current with emerging literature and evolving standards in hematopoietic stem cell transplantation pharmacotherapy.


Required:

  • Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy.
  • Current pharmacist license in Florida or eligibility for licensure.
  • Oncology and BMT experience
  • Comprehensive understanding of chemotherapy, immunosuppressants, antifungals, antivirals, and other transplant-related medications.
  • Excellent clinical judgment, communication, and organizational skills.


Preferred:

  • Board Certification in Oncology Pharmacy (BCOP).
  • Residency trained
  • Experience working within an accredited transplant program (FACT, CIBMTR, or equivalent).
  • Familiarity with Epic or other EHR systems and oncology/BMT clinical decision support tools.
