**Senior NP Clinical Director** Skilled Nursing/Long Term Care**
LOCATION: Multi State oversite(You will travel)
SHIFT: Day

Job Summary:
We are seeking a dynamic and experienced Senior Clinical Director to lead and enhance our clinical care initiatives. This leadership role will oversee nurse practitioners, physician assistants, RN case managers, and administrative staff. The ideal candidate will be passionate about improving patient outcomes, driving excellence in service, and developing strong clinical teams. This position supports leadership in the training, development, and coaching of staff to elevate standards of care across a Medicare Advantage institutionalized special needs population.
Key Responsibilities:
· Lead the transition from traditional care to a model focused on disease prevention, chronic condition management, and comprehensive primary care.
· Support interdisciplinary collaboration to enhance communication and care coordination across clinical teams.
· Ensure that care provided is cost-effective, high quality, and tailored to the needs of each patient.
· Direct and indirectly supervise clinical and administrative team members.
· Coach, develop, and train team members to uphold clinical excellence and customer service.
· Support continuous quality improvement efforts and ensure adherence to best practices in care delivery.
· Maintain clear, professional communication with patients, providers, and support teams.
· Perform other duties as assigned.

Requirements:
· Mid-Level Practitioner (Nurse Practitioner or Physician Assistant).
· Graduate of an accredited nursing or mid-level practitioner program.
· Current licensure and certifications (RN/NP/PA, CPR).
· Minimum 3 years of experience managing a clinical team.
· At least 5 years of experience working with long-term care or chronically ill patients.
· Strong familiarity with Medicare, Medicaid, ISNP, nursing facilities, and managed care models.
· Knowledge of HEDIS and STARs metrics is preferred.
· Competent in using Personal Protective Equipment (PPE)
· Travel required (reliable transportation, valid driver’s license and auto insurance necessary)
