📍 Gainesville, FL | Full-Time | Monday–Friday | No Regular Weekends

🕐 Shifts: 12:00 PM – 8:30 PM (flexible to 11:00 AM – 7:30 PM)

☀️ Only 2 weekend days/month required (does not have to be consecutive)

About the Role

We’re seeking a skilled RN Case Manager to coordinate and evaluate the medical necessity, level of care, and discharge planning for patients. You’ll work collaboratively with physicians, patients, families, and payors to ensure quality outcomes and smooth transitions of care.

Key Responsibilities

• Perform comprehensive clinical assessments and facilitate care plans
• Coordinate discharge plans and ensure continuity across the care continuum
• Participate in interdisciplinary team rounds and utilization review
• Communicate with insurance providers to obtain authorizations and manage denials
• Educate patients and families about health management and care transitions
• Accurately document in MIDAS and apply InterQual criteria

📋 Requirements

✔️ 3+ years recent acute care RN Case Management experience

✔️ Strong critical thinking and understanding of medical complexities

✔️ Active FL RN license or compact (with 60-day FL licensure deadline post-move)

✔️ BLS (AHA) certification within 30 days of hire

✔️ ASN required, BSN preferred

✔️ Case Management or Utilization Review certification a plus
