Job Title:Clinical Pharmacist – Bone Marrow Transplant (BMT)

Position Summary
The Clinical Pharmacist – Bone Marrow Transplant (BMT) is an integral member of the interdisciplinary transplant care team at a full-service, acute care teaching hospital. This pharmacist provides comprehensive pharmaceutical care to adult and/or pediatric BMT patients, ensuring safe, evidence-based, and patient-centered medication management throughout the transplant continuum. The role emphasizes clinical expertise in oncology, immunosuppression, antimicrobial stewardship, and supportive care, while also supporting education, research, and quality improvement initiatives.
Key Responsibilities:
Clinical Care
- Provide direct clinical pharmacy services to BMT patients in inpatient and/or outpatient settings.
- Review, verify, and optimize medication therapy for conditioning regimens, immunosuppressive therapy, chemotherapy, antimicrobials, and supportive care agents.
- Monitor and adjust therapy based on pharmacokinetics, organ function, drug interactions, and adverse effects.
- Participate in daily interdisciplinary rounds and contribute to treatment planning and clinical decision-making.
- Provide patient and caregiver education on transplant-related medications, adherence, side-effect management, and transitions of care.
Safety & Quality
- Ensure compliance with chemotherapy verification standards, institutional protocols, and regulatory requirements.
- Actively identify, prevent, and resolve medication-related problems and adverse drug events.
- Support antimicrobial stewardship and infection prevention initiatives within the BMT population.
- Participate in quality improvement projects related to transplant outcomes, medication safety, or operational efficiency.
Education & Teaching
- Serve as a preceptor for pharmacy residents, students, and trainees as part of the hospital’s teaching mission.
- Provide education to physicians, nurses, and other healthcare professionals on BMT pharmacotherapy.
Research & Professional Practice
- Contribute to clinical research, protocol development, and guideline implementation related to BMT and oncology pharmacy practice.
- Maintain competency through continuing education and active engagement in professional development.
Operational Responsibilities
- Support pharmacy operations as needed, including order verification and coverage during rotating weekends.
- Collaborate with pharmacy leadership to ensure consistent, high-quality service delivery.
Qualifications:
Required
- Doctor of Pharmacy (PharmD) from an ACPE-accredited school of pharmacy
- Active pharmacist license (or eligibility) in the practicing state
- Completion of a PGY1 Pharmacy Residency
- Strong clinical knowledge in oncology, hematology, and immunosuppressive therapies
- Ability to work day shift with rotating weekend coverage
Preferred
- PGY2 Residency in Oncology or equivalent BMT/oncology experience
- Board Certification (BCOP) or eligibility
- Prior experience in a BMT or hematology/oncology practice setting
- Experience in a teaching hospital or academic medical center
