Position Summary
The Speech-Language Pathologist (SLP) provides high-quality speech therapy services in accordance with physician orders to support patient recovery and communication function. This role collaborates closely with patients, families, and interdisciplinary healthcare teams to deliver compassionate, effective care across home health settings while maintaining regulatory and clinical standards.
Key Responsibilities
- Deliver speech therapy evaluations and treatment plans based on physician orders and patient needs
- Educate patients and family members to support therapy goals and care plans
- Maintain accurate, timely clinical documentation in compliance with regulatory standards
- Coordinate care with physicians, clinical managers, and interdisciplinary team members
- Demonstrate strong time management and independent workflow organization
- Utilize conflict resolution and customer service best practices in patient interactions
- Maintain knowledge of applicable state laws, home health regulations, and accreditation standards
- Leverage community resources and referral systems to support patient outcomes
- Ensure patient safety in both office and home environments
- Uphold professional appearance and conduct standards

- Capacity to work effectively in diverse environments including patient homes
Physical & Work Requirements
- Moderate to high physical activity including walking, standing, bending, lifting up to 50 pounds, and driving
- Ability to perform fine motor tasks and operate clinical and office equipment
- Willingness to travel to patient homes, including rural or remote areas
- Adaptability to varied home environments while maintaining safety standards
Qualifications
Education & Training
- Graduate of an accredited Speech-Language Pathology program
- Master’s degree preferred; Bachelor’s degree acceptable with completion (or in progress) of Clinical Fellowship Year (CFY) in accordance with ASHA guidelines
Experience
- 0–1 year of related clinical experience preferred (home health or healthcare setting)
Licenses & Certifications (Required)
- Valid state license in Speech-Language Pathology
- Current Basic Life Support (BLS) certification
- Valid driver’s license and proof of automobile insurance
