JOB DESCRIPTION SUMMARY

The Licensed Clinical Social Worker (LCSW) provides emotional support, counseling, and advocacy to hospice patients and their families. The LCSW helps individuals navigate the emotional, social, and practical challenges associated with serious illness and end-of-life care. As a core member of the interdisciplinary hospice team, the LCSW collaborates with nurses, physicians, chaplains, and other care providers to ensure comprehensive and compassionate patient- and family-centered care

 

POSITION QUALIFICATIONS

·     EDUCATION:

o   Master’s degree in social work (MSW) from an accredited program.

·     EXPERIENCE:

o  Minimum of Three (3) years of experience in healthcare

o  Experience in healthcare, hospice, or a related industry preferred.

·     CERTIFICATION/LICENSE(S):

o  Current, active state licensure as a Licensed Clinical Social Worker (LCSW)

o  Valid Driver’s License


ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

·     Provide counseling and emotional support to patients and families, including anticipatory grief and bereavement care.

·     Facilitate family meetings to enhance communication, care planning, and emotional support.

·     Collaborate with the interdisciplinary team to align psychosocial care with the patient’s medical and spiritual needs.

·     Assist families in accessing resources, benefits, and support services (e.g., financial assistance, advance care planning, funeral arrangements).

·     Document assessments, interventions, and progress notes in compliance with regulatory and organizational standards.

·     Participate in the hospice bereavement program, offering ongoing support to families after the patient’s death.

·     Uphold ethical standards and confidentiality according to state, federal, and NASW guidelines.


KNOWLEDGE, SKILLS, AND ABILITIES

·     Strong understanding of end-of-life care principles, grief counseling, and family dynamics.

·     Excellent communication, active listening, and counseling skills.

·     Ability to work compassionately with patients and families facing emotional, spiritual, and practical challenges.

·     Knowledge of hospice regulations, including Medicare/Medicaid guidelines.

·     Strong organizational skills and ability to prioritize in a fast-paced environment.

·     Comfortable working independently and collaboratively as part of an interdisciplinary team.

·     Proficient with electronic health records and documentation standards.
