We are seeking a dedicated Nurse Practitioner to provide high-quality care to patients in post-acute and long-term care settings throughout the Fincastle, Virginia area.

What You'll Do
As a Nurse Practitioner, you will serve as a key clinical leader responsible for evaluating, diagnosing, treating, and coordinating care for patients in post-acute care environments.
Primary Responsibilities
• Conduct comprehensive patient assessments and evaluations.
• Develop, implement, and monitor individualized treatment plans.
• Perform routine patient rounds and follow-up visits.
• Write and manage medical orders, prescriptions, and care plans.
• Collaborate with physicians, nursing staff, rehabilitation teams, and Directors of Nursing to ensure continuity of care.
• Order, interpret, and evaluate diagnostic testing and laboratory results.
• Provide patient and family education regarding treatment plans and health management.
• Document patient encounters accurately and timely within the electronic medical record (EMR).
• Participate in interdisciplinary care planning and quality improvement initiatives.
• Ensure compliance with state, federal, and facility regulations.

Required Qualifications
✔ Active Nurse Practitioner License
✔ Active DEA Certification
✔ Strong clinical assessment and diagnostic skills
✔ Excellent communication and interpersonal abilities
✔ Ability to work independently while collaborating effectively with healthcare teams
✔ Strong organizational and time-management skills
✔ Passion for providing patient-centered care
Preferred Qualifications
✔ One or more years of Nurse Practitioner experience
✔ Background in:
Geriatrics
Internal Medicine
Emergency Medicine
Intensive Care Unit (ICU)
Hospitalist Medicine
Family Practice
General Practice
✔ Experience working within skilled nursing, post-acute, rehabilitation, or long-term care settings
