Job Summary
The Clinical Hospital Pharmacist is responsible for the safe, accurate, and efficient distribution and monitoring of pharmaceutical care within the hospital setting. This role involves verifying medication orders, compounding pharmaceuticals, monitoring patient drug therapies, and collaborating closely with the broader healthcare team. The ideal candidate will leverage clinical expertise to promote rational, cost-effective drug therapy selection while supervising daily pharmacy operations and technician workflows.
Major Responsibilities
Clinical Practice & Patient Safety
- Order Verification: Interpret, transcribe, and verify provider medication orders accurately and efficiently within the hospital’s electronic pharmacy information system.
- Therapeutic Monitoring: Review and monitor patient drug regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug selection and dosing.
- Clinical Consultations: Actively support clinical pharmacy services, including pharmacokinetic dosing, nutrition support (TPN), anticoagulant monitoring, pain management, renal dosing protocols, and IV-to-PO conversions.
- Intervention Documentation: Document all clinical activities, specialist consultations, and targeted provider interventions accurately and completely in accordance with department policy.
- Safety & Quality: Identify, manage, and report adverse drug reactions (ADRs) and medication errors to promote continuous quality improvement and patient safety.
Pharmacy Operations & Formulary Management
- Compounding & Dispensing: Safely and accurately compound and dispense medications, including sterile IV admixtures and hazardous medications, adhering to established regulatory safety standards.
- Formulary Compliance: Support and sustain the hospital formulary by minimizing non-formulary procurements, facilitating therapeutic interchanges, and promoting cost-effective, evidence-based medication therapy.
- Staff Supervision: Effectively oversee daily pharmacy operations and provide real-time clinical and operational guidance to pharmacy technicians and support staff.
- Drug Information Resource: Provide timely, evidence-based medication information and consultations to physicians, nursing staff, and patients.
Job Requirements
Education & Licensure
- Education: Bachelor of Science (B.S.) in Pharmacy or Doctor of Pharmacy (PharmD) from an ACPE-accredited School of Pharmacy required.
- Licensure: Active Pharmacist license in good standing with the Florida State Board of Pharmacy required.
Experience Preferences
- Minimum of one (1) year of recent hospital pharmacy experience is preferred.
- Successful completion of an ASHP-accredited PGY1 Pharmacy Residency will be accepted in lieu of hospital experience.
