JOB TYPE: Full-Time Permanent

We are seeking a compassionate Registered Nurse to join our hospice team as an Admissions RN. In this role, you will guide patients and families through the hospice admission process by completing comprehensive assessments, coordinating care, and providing education and support during a significant life transition.

Responsibilities

• Complete hospice admission assessments to determine eligibility and identify patient and family needs.
• Develop and initiate individualized plans of care in collaboration with physicians and the interdisciplinary team.
• Educate patients and caregivers about hospice services, medications, symptom management, and available resources.
• Coordinate admissions, equipment, and services to ensure a smooth transition into hospice care.
• Maintain accurate clinical documentation and communicate changes in patient status.
• Collaborate with physicians, referral sources, and the hospice team to deliver high-quality, patient-centered care.
• Participate in on-call rotation, interdisciplinary meetings, and quality improvement initiatives.
• Supervise hospice aides as appropriate.

Qualifications

• Current Registered Nurse (RN) license.
• Graduate of an accredited nursing program.
• Recent nursing experience in acute care, home health, hospice, or a related setting preferred.
• Valid driver's license, reliable transportation, and current auto insurance.
• Strong assessment, communication, organization, and critical thinking skills.
• Ability to work independently while providing compassionate, patient-centered care in home settings.
