Are you an experienced Nurse Practitioner who wants the opportunity to spend meaningful time with patients and practice primary care with a strong clinical support team?
We are partnering with a growing healthcare organization seeking an experienced Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) for its Great Falls, Montana clinic. This is an opportunity to provide comprehensive, relationship-based primary care to an older adult population in a model focused on quality, outcomes, prevention, and continuity of care, not simply patient volume.
As a Nurse Practitioner, you will manage your own panel of patients while working alongside a collaborative team that includes Medical Assistants/LPNs, clinic leadership, care coordination resources, and physician support. You will serve as the primary clinical provider for your patients, helping manage their overall health while coordinating specialty care, preventive services, chronic disease management, and transitions following hospitalization.
What You'll Do:
- Provide comprehensive primary care for an adult and senior patient population.
- Evaluate, diagnose, treat, and manage acute and chronic conditions.
- Conduct preventive care visits, Annual Wellness Visits, and health assessments.
- Develop individualized treatment and disease-management plans.
- Order and interpret diagnostic testing.
- Prescribe medications within Montana scope-of-practice guidelines.
- Educate patients regarding prevention, wellness, medications, and chronic disease management.
- Coordinate referrals and collaborate with specialists and other healthcare providers.
- Follow patients through hospitalizations and post-discharge care.
- Maintain accurate and timely clinical documentation.
- Support quality measures, risk adjustment, and appropriate HCC documentation.
- Participate in interdisciplinary care meetings and clinical improvement initiatives.
- Provide occasional telehealth or other visit modalities based on patient needs.
- Participate in a shared after-hours coverage rotation.
- Help support and mentor new clinicians or NP students when appropriate.
Requirements:
- Active Montana FNP or AGNP license
- Current CPR/BLS certification
- Montana prescriptive authority and DEA license
- 3–5+ years of independent primary care experience
- Strong experience managing patients with multiple chronic conditions
- Valid Montana driver's license
- Comfortable working collaboratively while independently managing a patient panel
Especially Valuable:
- Experience caring for Medicare or older adult populations
- Adult-Gerontology board certification
- Value-based care or population health experience
- HCC/risk-adjustment knowledge
- Care coordination or complex case management experience
- Experience helping grow or launch a medical practice
- Strong EHR documentation skills
- Interest in patient and community education
