Advanced Practice Provider – Physician Assistant / Nurse Practitioner in Family Medicine
Fulltime Day Shift
M-F 9am-5pm
Salary $150K base salary
Location: Brooklyn New York/Long Island City NY
Position Summary
We are seeking an experienced Physician Assistant or Nurse Practitioner to provide comprehensive, patient-centered care to pediatric, adolescent, and adult patients in clinic and community-based settings.
Key Responsibilities
- Perform comprehensive patient assessments, histories, physical exams, and well-care visits.
- Diagnose conditions and develop, implement, and monitor treatment plans.
- Prescribe medications and educate patients on proper use.
- Complete clinical documentation, medical forms, and EMR records.
- Coordinate referrals and ensure appropriate follow-up.
- Perform or assist with immunizations and phlebotomy as needed.
- Collaborate with physicians, specialists, and other healthcare professionals.
- Participate in peer reviews, case conferences, and quality improvement initiatives.
- Provide education on preventive care, chronic disease management, nutrition, family planning, medication management, and wellness.
- Promote health maintenance, disease prevention, and patient advocacy.
- Participate in patient outreach activities.
- Provide coverage at satellite locations, including Brooklyn and Queens, as needed.
- When working at satellite locations, provide guidance and oversight to medical assistants.
- Follow HIPAA, infection control, and organizational policies and procedures.
- Assist with the development and implementation of clinical protocols and forms.
Qualifications
- Current, unrestricted New York State PA or NP license.
- 2+ years of clinical experience preferred.
- Strong clinical assessment, communication, and organizational skills.
- Comfortable working collaboratively with physicians and multidisciplinary teams.
- Proficiency with EMR systems and standard computer applications.
- Spanish or additional language skills preferred.
- MUST be Board/DEA Certified and be an individual NYS Medicaid Provider.
Work Environment
The provider will work in clinic and community-based settings, including affiliated shelters and satellite locations. The role requires collaboration with the clinical team to provide high-quality care while maintaining appropriate infection control and patient safety standards.
