Patient Care Coordinator RN
Fulltime Day Shift
M-F 9am-5pm
Position Summary
The Patient Care Coordinator supports the delivery of coordinated, patient-centered care by proactively managing high-risk patients, developing individualized care plans, and collaborating with providers and internal teams to improve patient outcomes and care coordination.
Key Responsibilities
- Anticipate patient needs and complete pre-visit planning prior to appointments.
- Review daily appointments and prepare, distribute, and analyze patient rosters.
- Coordinate and document daily patient care conferences.
- Conduct proactive outreach to high-risk patients.
- Provide advanced care management to an identified high-risk patient population.
- Develop individualized care plans in collaboration with providers.
- Assist patients in establishing treatment goals and identifying potential barriers to care.
- Educate patients regarding their care plans and treatment goals.
- Accurately document care plans and patient interactions in the EMR.
- Participate in Quality Improvement (QI) team meetings.
- Collaborate with Benefits Counselors and Outreach staff to support appropriate scheduling and patient flow.
- Work with Health Information Management (HIM) leadership to ensure proper documentation procedures.
- Assist with additional duties, assignments, and special projects as needed.
Qualifications
- Current, valid New York State RN License.
- Prior clinical experience preferred.
- Administrative experience preferred.
- Strong organizational, communication, and care coordination skills.
- Experience with electronic medical records preferred.
Work Environment
The Patient Care Coordinator will work in a clinical setting with designated space for patient care planning and administrative responsibilities. The position requires collaboration with providers and multidisciplinary teams to support effective patient care and coordination.
EMR Experience
Experience with EMR functions including patient demographics, registration, scheduling, scanning, triage, medical history/vitals, orders, immunizations, progress notes, reporting, laboratory information, medical summaries, and telephone encounters is preferred.
