Position Summary

We are seeking a compassionate and skilled Occupational Therapist (OT) to join our interdisciplinary PACE (Program of All-Inclusive Care for the Elderly) healthcare team serving older adults. The Occupational Therapist evaluates, plans, and implements individualized treatment programs designed to help participants develop, recover, or maintain the skills necessary for daily living, safety, and independence.

This position combines home-based safety assessments, skilled therapeutic interventions in a day-center setting, durable medical equipment (DME) coordination, and collaboration with an interdisciplinary care team. The OT plays an important role in helping older adults remain safely and independently in their homes and communities while improving their overall quality of life.


Key Responsibilities

  • Conduct comprehensive occupational therapy evaluations and functional assessments.
  • Develop individualized treatment plans based on participant needs, goals, and interdisciplinary care plans.
  • Provide skilled occupational therapy interventions to improve strength, mobility, coordination, functional performance, and activities of daily living.
  • Perform home safety assessments and recommend appropriate modifications, adaptive equipment, and safety strategies.
  • Evaluate, recommend, and coordinate durable medical equipment and other supportive services.
  • Educate participants, caregivers, and families regarding therapeutic techniques, adaptive strategies, and safety.
  • Monitor participant progress and modify treatment plans as clinically appropriate.
  • Collaborate closely with physicians, nurses, physical therapists, social workers, and other healthcare professionals.
  • Promote participant independence, dignity, safety, and quality of life.
  • Maintain accurate and timely clinical documentation in the electronic health record.
  • Participate in interdisciplinary meetings and contribute to comprehensive care planning.
  • Provide compassionate care in a variety of settings, including emotionally sensitive situations.


Qualifications

Required

  • Bachelor's, Master's, or Doctorate degree in Occupational Therapy from an accredited program.
  • Current NBCOT registration.
  • Current Colorado Occupational Therapist license or ability to obtain licensure before employment.

Preferred

  • 1–3 years of occupational therapy experience.
  • Experience working with older adults or geriatric populations.
  • Experience with home safety assessments, adaptive equipment, or DME.

Compensation & Benefits

  • $41.58–$46.00 per hour, based on experience.
  • $10,000 sign-on bonus.
