Job Type: Full-Time

Salary: $60,000–$64,000 per year, depending on experience.

Schedule: Onsite | Monday–Friday.

• Monday, Wednesday & Friday: 9:00 AM–5:00 PM
• Tuesday & Thursday: 10:00 AM–6:00 PM

We are seeking a compassionate and dedicated Social Worker to join a supportive housing team in the Bronx. This position is ideal for a Social Worker who is passionate about trauma-informed care, harm reduction, behavioral health, and helping individuals maintain stable housing.

The Social Worker will provide comprehensive assessments, individualized service planning, case management, therapeutic interventions, crisis support, and referrals to community resources.

Responsibilities

• Conduct comprehensive bio-psycho-social assessments addressing substance use, physical and mental health, family, relationships, education, employment, and social service needs.
• Develop individualized service plans with clear goals and outcomes focused on housing stability and overall well-being.
• Provide case management and support related to:
• Daily living skills
• Crisis intervention
• Financial management
• Mental health
• Substance use
• Public benefits and entitlements
• Coordinate services with tenants’ treatment providers and other professionals, with appropriate written consent.
• Connect tenants with community resources, treatment programs, and supportive services.
• Assist tenants with establishing household and community-based services that promote housing stability.
• Provide individual and group therapeutic interventions.
• Build supportive relationships with tenants using a trauma-sensitive, person-centered, harm reduction approach.
• Maintain accurate documentation of assessments, service plans, interventions, and progress.
• Perform other duties as assigned.

Qualifications:

Education – Master’s Degree Required in:

• Social Work (MSW)
• Mental Health Counseling (MHC)
• Public Administration (MPA)
• Public Health (MPH)
• Psychology (MA)
• Human Services (MS)
• Related field

Experience & Skills:

• Experience providing therapeutic interventions.
• Experience utilizing community resources.
• Experience working with individuals who have experienced homelessness and/or substance use.
• Strong case management, assessment, communication, and crisis intervention skills.
• Ability to work collaboratively with tenants, treatment providers, and community organizations.
• Commitment to trauma-informed and harm reduction principles.
