Location: Rural Retreat, VA

Salary:

Geriatric Physician

Older adults deserve care that protects independence, prevents avoidable decline, and treats the whole person—not a list of diagnoses. In this role, you will lead clinical decisions that keep patients safer at home, reduce crisis-driven care, and give families a clear plan they can trust. Your judgment will shape how preventative care and health management actually work for people living with complex, age-related conditions.

Performance Objectives

  • Complete timely, comprehensive geriatric assessments and care plans that address function, cognition, medications, goals of care, and social risk.
  • Reduce preventable hospitalizations and emergency visits by identifying early decline and coordinating interventions before a crisis.
  • Reconcile and simplify medication regimens, cutting high-risk polypharmacy and adverse drug events.
  • Partner with patients and families on advance care planning so treatment matches stated goals.
  • Guide interdisciplinary care so nursing, therapy, and care management execute one coherent plan.
  • Improve chronic-condition control and functional stability for a defined panel, using measurable follow-up.
  • Raise clinical quality by reviewing outcomes, closing care gaps, and standardizing geriatric protocols.

Environment & Resources

You will practice in a small, early-stage elder-care organization focused on preventative care, health management, and treatment for older adults. You will work closely with nurses, care managers, and allied clinicians, with direct access to leadership on clinical priorities. Expect a lean team, shared electronic records, and the authority to shape protocols rather than inherit a rigid playbook. You will report to clinical leadership and help define how physician time, panel size, and support staff are used.

Essential Qualifications

  • MD or DO with active license and board certification or eligibility in geriatrics, internal medicine, or family medicine with substantial geriatric practice.
  • Demonstrated success managing multimorbidity, dementia, frailty, and polypharmacy in outpatient or post-acute settings.
  • Proven ability to lower avoidable acute utilization through proactive care planning.
  • Skill leading interdisciplinary teams and communicating clearly with patients and families.
  • Comfort practicing with limited infrastructure and improving clinical workflows as the practice grows.

Role Selling Points

  • Clinical influence is real: you help design how elder care is delivered, not only fill a schedule.
  • The mission is prevention and function, not volume-driven encounters.
  • A small team means shorter paths from insight to protocol change.
  • Your work is visible in independence preserved, medications simplified, and crises avoided.

If you want your clinical skill to change how older adults live—not only how they are treated—apply with a brief note on a geriatric outcome you improved and how you did it.
