Compensation: $87,000 - $131,000 annually.

We are seeking a dedicated and compassionate Social Work Care Manager to join our Care Management team. This full-time, day shift position is Monday-Friday, 9 am-5 pm, with rotating weekend coverage (Saturday & Sunday).

About the Role:

As a Social Work Care Manager, you will play a crucial role in ensuring patients receive the appropriate support services for a smooth and successful discharge. You'll conduct comprehensive assessments, identify high-risk patients, coordinate care, and facilitate transitional planning to prevent readmissions and improve patient outcomes.

Responsibilities:

• Conduct comprehensive initial assessments upon admission, screening all patients for high-risk indicators.
• Provide ongoing reassessments of patient needs throughout their hospital stay.
• Coordinate multidisciplinary care within and between hospitals.
• Facilitate all transitional planning activities, including:
• Patient and caregiver education
• Arranging aftercare services
• Engaging with commercial payers
• Addressing root causes for high utilizers
• Communicate and collaborate effectively with the healthcare team.
• Contribute to departmental goals of reducing length of stay, preventing readmissions, reducing denials, and improving patient satisfaction.
• Maintain accurate and timely documentation according to department requirements.

Qualifications:

• Master of Social Work (MSW) degree from a CSWE-accredited School of Social Work.
• Licensed Social Worker (LMSW/LCSW) in New York State.
• Two years of acute care hospital experience or related setting preferred.
• NYS SCREEN Certification (must be obtained within the first 3 months of employment).
