Position Overview:
The Director of Case Management provides strategic and operational leadership for all aspects of the case management function in a large acute care hospital. This includes oversight of care coordination, discharge planning, resource utilization, and compliance with regulatory standards. The Director is a highly visible leader who collaborates across interdisciplinary teams to ensure quality outcomes, patient satisfaction, and cost-effective care delivery.
This role requires strong executive presence, excellent communication skills, and the ability to lead high-performing teams in a complex healthcare environment. The ideal candidate is a seasoned registered nurse with extensive leadership experience and a demonstrated ability to drive performance improvement initiatives while building trust across stakeholders.
Key Responsibilities:
- Direct all departmental operations including case management models, staffing, onboarding, performance evaluation, and staff development.
- Manage departmental resources to align with productivity benchmarks and quality targets.
- Ensure timely and compliant execution of care coordination and discharge planning processes.
- Collaborate with senior leadership to set and meet case management goals, focusing on clinical effectiveness and financial performance.
- Analyze department performance metrics and lead quality improvement initiatives to optimize outcomes.
- Ensure compliance with internal policies, standard operating procedures, and all regulatory requirements.
- Build collaborative relationships with key stakeholders, including medical staff, nursing leadership, compliance teams, and legal advisors.
- Provide leadership in times of change, managing competing priorities and ensuring seamless communication across teams.
- Maintain a patient-centered approach that promotes continuity of care and efficient resource use.
- Perform other duties as assigned in alignment with organizational goals.
Qualifications:
- Minimum 2 years of recent (within the last 2 years) experience as a Director of Case Management in a large acute care hospital is required.
- Bachelor’s degree in Nursing is required.
- Master’s degree in Nursing, Health Administration, or Business Administration is preferred.
- Active RN licensure is required (LCSW or other non-nursing credentials not accepted for this role).
- Proven ability to lead high-functioning teams and work collaboratively with C-suite and executive stakeholders.
