Nurse Practitioner- Primary Care

POSITION TYPE: Primary Care Nurse Practitioner - New Clinic!

LOCATION: Twin Falls, Idaho

SHIFT: 8am-5pm (M-F)

As a Primary Care Nurse Practitioner, you will take a lead role in delivering personalized, high-quality care to a panel of senior patients in a primary care setting. You will work collaboratively with a supportive care team, including Medical Assistants or LPNs, a Front Office Coordinator, Clinic Manager, and Medical Director. You will provide holistic, coordinated, and relationship-based care. This position offers the opportunity to make a meaningful impact through high-touch, team-oriented healthcare in a patient-centered environment.

Key Responsibilities

• Provide full-scope primary care (acute, chronic, preventive)
• Conduct Annual Wellness Visits and Health Risk Assessments
• Manage patient panels and coordinate care with support staff
• Order/interprets labs, perform screenings, and provide education
• Make referrals, complete prior authorizations, and post-discharge follow-ups
• Deliver care via in-clinic, telehealth, or occasional home visits
• Ensure accurate HCC coding and meet quality benchmarks
• Maintain timely and complete charting in the EHR
• Leads clinic operations, team meetings, and community outreach
• Mentor new providers and precept students, as needed
• Delivers high-quality patient care and fosters an exceptional patient experience, as demonstrated by strong patient satisfaction scores.
• Effectively collaborates and communicates with all members of the care team to ensure patient needs are met seamlessly during clinic visits and beyond, supporting a smooth and coordinated care journey.
• Rotate after-hours call with other clinicians

Requirements

• Active FNP or AGNP license in Idaho
• DEA license and prescriptive authority
• 
  
• Strong EHR and Microsoft Office skills
• CPR/BLS certification
• Proficiency in MS Word, Excel & PowerPoint
• Valid Idaho driver’s license

Preferred

• Adult-Gerontology certification
• Experience with value-based care and new practice growth
• Background in patient education and care coordination
