Position Summary

We are seeking an experienced, leadership-minded Remote Nurse Practitioner to guide clinical operations, oversee patient outreach, and deliver high-touch Chronic Care Management. In this role, you will lead a multidisciplinary care team, manage clinical escalations, and build tailored care plans for complex patient populations.

Key Responsibilities

  • Clinical Operations & Escalations: Build individualized care plans for high-complexity patients and manage clinical escalations to ensure high-quality, safe care.
  • Team Leadership & Oversight: Supervise nurse outreach workflows, guide clinical operations, and partner with care coordination leadership on team onboarding and ongoing staff development.
  • Interprofessional Collaboration: Work closely with medical directors, nurses, care coordinators, specialists, and pharmacists to optimize patient outcomes.
  • Patient Engagement: Use motivational interviewing techniques to advocate for, support, and empower members in managing chronic conditions via phone and text outreach.
  • Documentation & Quality: Maintain timely, accurate case notes and documentation within care management and Electronic Health Record (EHR) platforms.

Required Qualifications

Licensure & Certifications

  • Active, unrestricted Nurse Practitioner (NP) license in Texas (ability to obtain additional state licenses as needed).
  • Current national board certification (ANCC, AANPCB, or equivalent) aligned with your clinical specialty.
  • Graduate of an accredited professional nursing program (CCNE or ACEN accredited at the time of graduation).

Experience & Skills

  • Leadership: 5+ years of progressive clinical leadership experience managing healthcare teams.
  • Remote & Tech Proficiency: 2+ years of dedicated remote/telehealth work experience; comfortable using electronic care management tools, text messaging, and phone platforms.
  • Clinical Domain: Proven experience in Chronic Care Management (CCM) and complex care delivery.
  • Patient Populations: Demonstrated experience serving Veterans, Medicaid, and/or Dual-Eligible populations.
  • Communication & Engagement: Mastery of motivational interviewing, strong problem-solving skills, and exceptional written and verbal communication.
  • Workplace Setup: Dedicated, private home office suitable for remote work and HIPAA compliance.

Preferred Qualifications

  • Multistate Licensure: Active licensure in one or more additional states (e.g., LA, MS, AR, OK, MO, KS, NE, UT, WY, CO).
  • Specialties: FNP, Adult NP, PMHNP, or Clinical Nurse Specialist (CNS).
  • Language: Bilingual fluency in Spanish.
  • Hardware: Experience using macOS / Apple computer systems (e.g., MacBook).
