Physical Therapist (PT) – Home Health
Position Summary
We are seeking a compassionate and motivated Physical Therapist to provide skilled therapy services to patients in their homes. The Physical Therapist is responsible for evaluating patients, developing individualized treatment plans, improving mobility and function, and helping patients achieve their highest level of independence and quality of life.
Full-Time, Part-Time, and PRN opportunities available.
Responsibilities
- Perform comprehensive physical therapy evaluations and assessments.
- Develop, implement, and modify individualized treatment plans based on patient needs and goals.
- Provide therapeutic interventions to improve strength, mobility, balance, safety, and functional independence.
- Educate patients and caregivers on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
- Assess for adaptive equipment and durable medical equipment needs.
- Monitor patient progress and adjust treatment plans as appropriate.
- Collaborate with physicians, nurses, caregivers, and other healthcare professionals to coordinate care.
- Complete accurate and timely clinical documentation.
- Participate in care planning, discharge planning, and interdisciplinary team meetings.
- Maintain compliance with all applicable regulations and professional standards.
Qualifications
Required
- Current Physical Therapist (PT) license in the state of practice.
- Graduate of an accredited Physical Therapy program.
- Current CPR certification.
- Valid driver's license, reliable transportation, and current auto insurance.
- Strong clinical, communication, and organizational skills.
Preferred
- Minimum of one year of Physical Therapy experience.
- Home health experience preferred.
- Experience working with adult and geriatric populations.
- Bilingual abilities are a plus.

Physical Requirements
